You may send us an email to clevercare-ezsupport@ccmapd.com or to your provider relations representative with your completed agreement. Your account will be created within 5 business days or sooner. Providers, due to plan roll-over transactions happening with CMS right now, EZ-Net may appear to only show effective dates in 2023.
